SHOP operates the Care Pathways Clinic at Cedar in collaboration with Penn Medicine physicians. Located at HUP Cedar Hospital, 501 S. 54th Street, Medical Office Building, Ground Floor, Suite 26, the clinic is open on Mondays from 4:00–6:00 PM. The clinic welcomes both insured and uninsured patients and provides physical examinations, chronic disease care, medication prescription refills, and high blood pressure management. Walk-ins are welcome.
